What should I bring to my appointment?

Please arrive 15 minutes prior to your scheduled appointment time with the following information available:

  • Valid insurance card
  • Driver’s license
  • List of current medications
  • Relevant test results
  • Referral from your primary care physician, if applicable
  • X-rays from the referring physician, if applicable
